Getting Ready for TEAM: How Carecierge Is Preparing to Support Hospitals Nationwide

The new CMS TEAM Model will change how over 700 hospitals deliver post-surgical care. Here’s how Carecierge is gearing up to help providers succeed.

In July 2025, CMS finalized one of its most significant mandatory payment models to date: the Transforming Episode Accountability Model (TEAM). Set to launch on January 1, 2026, TEAM requires hundreds of hospitals to coordinate care for Medicare patients for 30 days after major surgical procedures—including hip replacements, spinal fusions, and coronary bypasses. Bonuses and penalties will be tied to outcomes, costs, and patient-reported experience. At Carecierge, we’re throwing our hat in the ring. After supporting over a dozen dementia providers under GUIDE, we’re now building out our platform to meet TEAM’s complex requirements—so hospitals don’t just comply, they perform.

The TEAM Model marks a turning point in CMS innovation. Hospitals will be held accountable not just for what happens in the operating room, but for everything that follows—home health, skilled nursing, rehab, readmissions, and even how patients say they’re feeling.

- Performance will be judged based on outcomes and spending

- Patient-reported outcomes (PROs) are no longer optional—they're required
